Recombivax HB is recombinant hepatitis B surface antigen vaccine. Prevention of HBV infection through universal childhood immunization has dramatically reduced new HBV infections in the US. Adult vaccination recommended for risk factors (healthcare workers, hemodialysis, MSM, IV drug users, sexual partners of HBV+, diabetes, chronic liver disease, etc.) or for any unvaccinated adult ≤59 years.
How Recombivax HB Works:
HBV causes acute and chronic hepatitis. Chronic infection can lead to cirrhosis and hepatocellular carcinoma. Vaccine produces antibodies to HBsAg (hepatitis B surface antigen) — protective immunity.
Recombivax HB contains recombinant HBsAg produced in yeast cells. Three doses (0, 1, 6 months) typically produce protective antibody levels in 95%+ of recipients. Birth dose critical to prevent perinatal HBV transmission from HBV+ mothers.
Form and use:
Recombivax HB given as IM injection in deltoid (or anterolateral thigh in infants). Standard 3-dose series: 0, 1, and 6 months. Higher dose for hemodialysis patients. Alternative schedules for certain populations.
Generic availability:
No ‘generic’ for vaccines. Other HBV vaccines: Engerix-B (3-dose, interchangeable with Recombivax HB), Heplisav-B (2-dose adjuvanted vaccine for adults — higher seroconversion, recommended for elderly and diabetic patients with poor response to traditional vaccines), PreHevbrio (3-antigen adult vaccine). Combined Twinrix (HAV + HBV).
Warnings:
Common: local injection site reactions, fatigue, headache. Rare: severe allergic reactions, alopecia (rare). Pregnancy: safe; recommended in pregnant women without immunity who are at risk.
$60–$120/dose. Insurance typically $0 (ACA preventive). VFC free for kids. Health departments often free for risk factors.
Recombivax HB and Engerix-B are similar 3-dose recombinant vaccines, interchangeable. Heplisav-B is a 2-dose adjuvanted vaccine for adults ≥18 — higher seroconversion rates, recommended for diabetic patients and elderly with poor response to traditional vaccines, but more expensive without insurance.
Yes — CDC recommends HBV vaccination for ALL children, and ACIP now recommends HBV vaccination for all adults aged 19–59 years (and adults ≥60 with risk factors). Hepatitis B birth dose remains critical.
Combined HAV + HBV vaccine for adults — useful for travelers needing both vaccines. Three-dose series convenient.
Recombinant HBV vaccine is safe in pregnancy and recommended for at-risk pregnant women. Babies born to HBV-positive mothers receive birth dose plus hepatitis B immunoglobulin (HBIG) immediately after birth.
Post-vaccination anti-HBs testing recommended for healthcare workers, hemodialysis patients, immunocompromised, and chronic HBV-exposed individuals. Routine testing not needed for most.
