How  Prevymis Tablets Works:

CMV reactivation after transplant causes significant morbidity. Historically managed with preemptive valganciclovir (toxic, myelosuppressive, resistance-prone) or universal prophylaxis with valganciclovir.

Letermovir targets CMV-specific viral terminase complex — essential for viral DNA packaging. Highly specific antiviral with minimal host toxicity. No effect on cellular DNA synthesis, no myelosuppression. Doesn’t share resistance pathways with ganciclovir.

Form and use:

Prevymis 480 mg once daily by mouth from transplant day 0 (or up to day 28) through day 100 post-HSCT. Or 480 mg daily for 200 days post kidney transplant (D+/R-).
