It is given as a subcutaneous injection — typically once or twice daily depending on indication. The coagulation cascade is a series of chemical reactions that form blood clots by converting fibrinogen into fibrin, with Factor Xa playing a critical role in producing thrombin. Special dose adjustments are required for individuals with poor kidney function, and anti-Xa blood monitoring is utilized for specific populations like pregnant, obese, or renal-impaired patients.

Warnings:
Lovenox carries a prominent boxed warning regarding the risk of spinal or epidural hematomas during spinal punctures or neuraxial anesthesia, which can result in permanent paralysis. It also carries warnings for serious bleeding and hemorrhage, especially in individuals who already possess bleeding risk factors. Additionally, patients may experience regular thrombocytopenia or a specific drug-induced form called heparin-induced thrombocytopenia, as well as elevated potassium levels. Finally, due to the risk of prosthetic heart valve thrombosis, the medication is not recommended for clot prevention in patients with artificial heart valves.

Lovenox is partly cleared by the kidneys. In patients with creatinine clearance <30 mL/min, the dose is reduced (typically 40 mg once daily for prophylaxis instead of standard; 1 mg/kg once daily for treatment instead of twice daily). In end-stage renal disease, unfractionated heparin is sometimes preferred. Your prescriber will adjust dosing for your kidney function.
