Lexiva (fosamprenavir) is an oral HIV-1 protease inhibitor used as a component of antiretroviral therapy. It is the prodrug of amprenavir and must be boosted with ritonavir for adequate antiviral activity. HIV guidelines now prefer integrase-inhibitor-based regimens for new starts; Lexiva is mostly historical except in patients already stable on it.
How Lexiva Works:
HIV-1 creates its essential proteins as a single large polyprotein, requiring a viral enzyme called protease to cut it into the individual functional pieces needed to form new infectious viruses. Fosamprenavir is converted within the body into active amprenavir, which binds directly to the HIV-1 protease and blocks this cutting process, resulting in non-infectious, immature virus particles instead. Because amprenavir is rapidly broken down by the liver enzyme CYP3A, it is co-administered with ritonavir, a CYP3A inhibitor. This combination safely boosts and maintains adequate drug levels in the body to allow for convenient once- or twice-daily dosing schedules.
Form and use:
Lexiva is available as oral tablets, which can be taken with or without food, and as an oral suspension, which must be taken on an empty stomach. The medication is typically taken alongside a ritonavir boost, with standard dosing tailored to a patient’s prior treatment history. For treatment-experienced patients, the typical regimen is 700 mg taken twice daily with 100 mg of ritonavir twice daily. For treatment-naïve patients, a once-daily dose of 1400 mg is used in combination with 100 mg to 200 mg of ritonavir once daily.
Generic availability:
Generic fosamprenavir is fully available in the United States at a significantly lower cost compared to the brand-name medication, Lexiva. Modern HIV clinical treatment guidelines generally favor alternative integrase-inhibitor-based regimens, such as Biktarvy or dolutegravir-based options, for patients who have never received treatment before. When a protease inhibitor is specifically required, healthcare providers typically prefer boosted darunavir options like Prezista with ritonavir, Prezcobix, or Symtuza. This preference is due to darunavir possessing a superior and higher genetic barrier to viral resistance compared to fosamprenavir.
Warnings:
Lexiva carries serious warnings for severe skin reactions like Stevens-Johnson syndrome, and because it contains a sulfa moiety, patients with known sulfa drug allergies may experience cross-reactivity. It can also cause liver toxicity, particularly in individuals co-infected with Hepatitis B or C, as well as metabolic changes like hyperglycemia, diabetes, hyperlipidemia, and fat redistribution. Additional risks include increased bleeding in hemophilia patients, kidney stones, and immune reconstitution syndrome. Furthermore, because amprenavir acts as both a substrate and inhibitor of CYP3A, ritonavir boosting significantly amplifies the risk of dangerous drug interactions.

The ritonavir dose used for boosting (100–200 mg) is much lower than antiviral doses; ritonavir at boosting doses has no significant antiviral effect itself.
Fosamprenavir contains a sulfonamide moiety. Patients with sulfa allergy may have cross-reactivity. Tell your HIV provider about any sulfa allergy. Severe rash, including Stevens-Johnson syndrome, has been reported with Lexiva — any new rash should be evaluated promptly.
Yes — many. Amprenavir is a CYP3A substrate AND inhibitor, and ritonavir is a strong CYP3A inhibitor. This combination affects many other medications (statins, certain antifungals, antiarrhythmics, anticonvulsants, ergot alkaloids, and more). Some combinations are contraindicated. Always tell every prescriber and pharmacist that you take Lexiva with ritonavir.
Yes. Medicare Part D beneficiaries can typically qualify, especially if you face specialty-tier copays. State ADAP programs may also coordinate with Medicare for HIV-treatment patients.
