Jevtana (cabazitaxel) is a second-generation taxane chemotherapy used for metastatic castration-resistant prostate cancer (CRPC) after docetaxel failure. It was designed to overcome the P-glycoprotein-mediated efflux mechanism that contributes to docetaxel resistance.
How Jevtana Works:
Taxanes bind to and stabilize microtubules, preventing the dynamic reorganization required for mitosis, which leads to cell-cycle arrest and cell death. While docetaxel is the first-line taxane for metastatic CRPC, patients often develop resistance via P-glycoprotein efflux pumps. Cabazitaxel is a semi-synthetic taxane designed to be a poor substrate for these pumps, allowing it to remain effective in docetaxel-resistant cancer cells. When combined with prednisone, it produces a meaningful overall survival benefit compared to older treatments like mitoxantrone.
Form and use:
Jevtana is administered as an intravenous infusion over 1 hour every 3 weeks, with a typical dose of 25 mg/m² or 20 mg/m² depending on patient tolerability. It is given concurrently with oral prednisone at a dose of 10 mg daily throughout the treatment period. Premedication with an antihistamine, H2 blocker, and corticosteroid is required 30 minutes before the infusion to prevent severe hypersensitivity reactions. Additionally, G-CSF prophylaxis is often recommended for patients undergoing this regimen.
Generic availability:
Generic cabazitaxel is available in the United States at a substantially lower cost than the brand-name Jevtana. For post-docetaxel metastatic CRPC, other treatment alternatives include hormonal therapies like enzalutamide and abiraterone, as well as radium-223 for symptomatic bone metastases. Additionally, lutetium-177-PSMA is available for PSMA-positive disease, while various PARP inhibitors can be utilized for cases involving BRCA mutations or homologous recombination repair deficiencies.
Warnings:
Jevtana carries a boxed warning for severe and fatal neutropenia, requiring weekly complete blood counts during the first cycle and avoidance if neutrophil counts are too low. It also carries a boxed warning for severe hypersensitivity reactions, making proper premedication mandatory. Other significant warnings associated with the medication include severe gastrointestinal symptoms such as hemorrhage and perforation, renal failure, urinary disorders like cystitis, interstitial lung disease, hepatic impairment, and embryo-fetal toxicity.

Concurrent prednisone 10 mg/day is part of the standard cabazitaxel regimen for CRPC. Prednisone has its own modest anti-tumor activity in CRPC (suppresses adrenal androgens) and reduces some chemotherapy side effects. The combination was used in the registration trials.
PROSELICA was a phase 3 non-inferiority trial showing 20 mg/m² cabazitaxel is non-inferior in survival to 25 mg/m² but with substantially less toxicity (neutropenia, febrile neutropenia diarrhea). Many oncologists now use 20 mg/m² as the starting dose, especially in older or frailer patients. The choice between 20 and 25 mg/m² is individualized.
Yes. Jevtana is typically billed through Medicare Part B (provider-administered IV infusion), which covers most of the cost; the PAP can help with copays or for patients without insurance.
