Isentress HD (raltegravir, 600 mg tablets) is the once-daily formulation of raltegravir — an integrase strand transfer inhibitor (INSTI). The HD (high-dose) formulation provides equivalent antiviral activity to twice-daily standard Isentress with once-daily dosing convenience.
How Isentress HD Works:
HIV-1 replication requires integrase — a viral enzyme that splices the HIV DNA copy into the host cell’s chromosomes. Without integration, HIV cannot establish persistent infection. INSTIs block this critical step.
Raltegravir binds HIV integrase at the active site and prevents strand transfer — blocking integration of viral DNA into the host genome. The Isentress HD formulation delivers 1200 mg once daily (two 600 mg tablets) with pharmacokinetics that allow effective viral suppression. Raltegravir has a lower genetic barrier to resistance than dolutegravir or bictegravir.
Form and use:
Isentress HD is taken as two 600 mg tablets (1200 mg total) by mouth once daily, with or without food. It must be combined with other antiretrovirals (typically FTC + tenofovir). It is for treatment-naïve adults and adolescents (≥40 kg). Patients with prior INSTI resistance or who require rifampin should NOT use Isentress HD — they should use standard twice-daily Isentress.
Generic availability:
There is no generic Isentress HD in the U.S. as of 2026. Newer INSTIs include dolutegravir (Tivicay; also in Dovato, Triumeq, Juluca), bictegravir (only in Biktarvy). HIV guidelines now generally prefer dolutegravir-based or bictegravir-based regimens for new starts due to higher resistance barriers and convenient once-daily fixed-dose combinations.
Warnings:
Isentress HD carries the same warnings as standard Isentress, including severe skin reactions (Stevens-Johnson syndrome, TEN, DRESS), hepatotoxicity (especially with HBV/HCV co-infection), depression/suicidality (rare), immune reconstitution syndrome, and rhabdomyolysis (rare). Isentress HD is CONTRAINDICATED with rifampin (which substantially reduces raltegravir levels — use standard Isentress instead at increased dose if rifampin is needed). Aluminum- and magnesium-containing antacids should be avoided (separate by at least 4 hours).
Isentress HD costs approximately $2,000–$2,400 per 30-day supply. Through AffordMyPrescriptions, qualifying patients receive Isentress HD at no medication cost — our $69.95 monthly fee covers full advocacy and program management.
Same active ingredient (raltegravir), different dose and dosing schedule. Standard Isentress is 400 mg twice daily. Isentress HD is two 600 mg tablets once daily. Isentress HD is for treatment-naïve adults/adolescents only; treatment-experienced patients with INSTI exposure typically use standard Isentress.
Isentress HD is not appropriate for treatment-experienced HIV patients with prior INSTI resistance, patients on rifampin (use standard Isentress instead with dose increase), or pediatric patients <40 kg. Discuss with your HIV provider whether HD or standard Isentress is right for you.
Newer INSTIs (dolutegravir, bictegravir) have higher genetic barriers to resistance and are available in convenient once-daily single-tablet regimens (Biktarvy, Triumeq, Dovato). Raltegravir-based regimens (including Isentress HD) are still effective and useful in specific situations, but are no longer first-line for most treatment-naïve patients in current HIV guidelines.
Avoid aluminum- and magnesium-containing antacids — they significantly reduce raltegravir absorption. If you need an antacid, calcium carbonate antacids (Tums) are acceptable. H2 blockers (famotidine) are also OK. Discuss timing with your pharmacist.
Yes. Medicare Part D beneficiaries can typically qualify, especially if you face specialty-tier copays. State ADAP programs may also coordinate with Medicare for HIV-treatment patients.
If denied, we explore alternatives — switching to standard Isentress, a dolutegravir-based regimen, a bictegravir-based regimen (Biktarvy), state ADAP, the manufacturer’s copay program for commercially insured patients, or independent foundations. If we cannot find a path, you won’t be charged our service fee.
