Understanding Isentress :

Isentress (raltegravir, RAL) was the first FDA-approved integrase strand transfer inhibitor (INSTI). It is used as a component of HIV-1 antiretroviral therapy in combination with other antiretrovirals — typically the FTC + tenofovir backbone.

How Isentress Works:

HIV-1 replication requires integrase — a viral enzyme that splices the HIV DNA copy into the host cell’s chromosomes. Without integration, HIV cannot establish persistent infection. INSTIs block this critical step.

Raltegravir binds HIV integrase at the active site and prevents strand transfer — blocking integration of viral DNA into the host genome. Raltegravir has a lower genetic barrier to resistance than newer INSTIs (dolutegravir, bictegravir) — meaning a single mutation can sometimes confer resistance. This makes raltegravir somewhat less forgiving of nonadherence than newer INSTIs.

Form and use:

Standard Isentress: 400 mg twice daily by mouth, with or without food. Isentress HD: 1200 mg (two 600 mg tablets) once daily for treatment-naïve adults and adolescents (≥40 kg). Pediatric formulations include chewable tablets and an oral suspension. Avoid antacids containing aluminum or magnesium (separate by at least 4 hours).

Generic availability:

There is no generic Isentress in the U.S. as of 2026. Newer INSTIs include dolutegravir (Tivicay; also in Dovato, Triumeq, Juluca), bictegravir (only in Biktarvy), and cabotegravir (in Cabenuva LAI and Vocabria oral). HIV guidelines now generally prefer dolutegravir-based or bictegravir-based regimens for new starts due to higher resistance barriers. Isentress is still used in select clinical scenarios (pediatric, certain drug interactions, pregnancy).

Warnings:

Isentress carries warnings for severe and potentially life-threatening skin reactions including Stevens-Johnson syndrome, toxic epidermal necrolysis, and DRESS — discontinue at the first sign of severe skin reaction or hypersensitivity. Other warnings include hepatotoxicity (especially with HBV/HCV co-infection), depression/suicidality (rare), immune reconstitution syndrome, rhabdomyolysis (rare; CK elevations), and significant drug interactions with antacids, rifampin (Isentress HD contraindicated; standard Isentress requires dose adjustment), and certain other agents.

Be careful. Aluminum- or magnesium-containing antacids (Mylanta, Maalox, etc.) significantly reduce raltegravir absorption — avoid these or separate dosing by at least 4 hours. Calcium carbonate antacids (Tums, Rolaids) and H2 blockers (famotidine) are generally OK. PPIs (omeprazole, etc.) are generally OK but discuss timing with your pharmacist.

Yes. Medicare Part D beneficiaries can typically qualify, especially if you face specialty-tier copays. State ADAP programs may also coordinate with Medicare for HIV-treatment patients.

Rifampin strongly induces UGT1A1 (the enzyme that metabolizes raltegravir), reducing raltegravir levels. Isentress HD is contraindicated with rifampin. With standard Isentress, the dose should be increased to 800 mg twice daily when on rifampin. Discuss with your HIV and TB providers.
