Felbatol (felbamate) is an antiepileptic used for severe refractory epilepsy — partial-onset seizures and Lennox-Gastaut syndrome in patients who have failed multiple other AEDs. Use is restricted due to serious risks of aplastic anemia and hepatic failure. Informed consent (acknowledgment of risks) is typically obtained before starting.
How Felbatol Works:
Felbamate has multiple proposed mechanisms — NMDA glutamate receptor antagonism (at the glycine site), enhancement of GABA-A activity, sodium channel blockade. The multi-mechanism action contributes to broad-spectrum efficacy across difficult-to-treat seizure types.
Felbamate is effective in refractory epilepsy including atonic seizures (drop attacks) in Lennox-Gastaut syndrome that often fail other AEDs. The serious safety concerns limit use to patients with truly refractory disease for whom the benefit/risk ratio favors felbamate.
Form and use:
Felbatol available as 400 mg and 600 mg tablets and oral suspension. Adult starting dose 1200 mg/day in 3–4 divided doses; titrate to 1800–3600 mg/day. Renal dose adjustment may be needed. Frequent CBC and LFT monitoring required.

Other warnings: suicidal ideation (AED class), embryo-fetal toxicity. Patient and prescriber awareness of risks is essential; informed consent obtained. Frequent CBC and LFT monitoring throughout therapy.

CBC every 1–2 weeks for the first 6 months, then less frequently. LFTs frequently in the first 6 months. Stop felbamate immediately if any blood count or liver enzyme abnormality.
