Epivir-HBV (lamivudine 100 mg) is an oral nucleoside analog antiviral used for chronic hepatitis B. The 100 mg dose for HBV is lower than the 300 mg dose used for HIV (Epivir). Lamivudine was one of the original HBV antivirals but has been largely replaced as first-line therapy due to high rates of HBV resistance.
How Epivir-HBV Works:
The Hepatitis B virus (HBV) replicates inside liver cells by utilizing a reverse transcriptase enzyme to copy its DNA genome. Lamivudine, a cytidine analog, works by getting phosphorylated inside these liver cells and incorporating itself into the growing viral DNA chain, which effectively terminates elongation and halts replication. However, the major drawback to this process is the rapid emergence of viral resistance, with 70% of patients developing the YMDD mutation in the HBV polymerase after five years of monotherapy. Because of this high resistance rate, current medical guidelines no longer recommend lamivudine as a first-line therapy, preferring alternatives like tenofovir or entecavir instead.
Form and use:
Epivir-HBV is administered orally as a single 100 mg tablet taken once daily, and it can be ingested with or without food. Because managing chronic HBV requires sustained viral suppression to prevent disease progression, treatment is typically a long-term commitment that often requires lifelong therapy for many patients. Additionally, because of how the medication is cleared by the body, a renal dose adjustment is required for patients with impaired kidney function.
Generic availability:
Generic lamivudine is widely available at a very low cost in both 100 mg and 150 mg strengths. Despite its availability, modern chronic HBV treatment guidelines prefer other first-line therapies, such as entecavir and tenofovir disoproxil fumarate (TDF), which is the standard choice for most adults. For patients who present with specific kidney or bone risk factors, tenofovir alafenamide is the preferred selection. Ultimately, lamivudine has lost its status as a first-line recommendation solely due to its high propensity for causing viral resistance.
Warnings:
Epivir-HBV carries a serious boxed warning regarding severe, acute HBV exacerbations that can occur if the medication is discontinued, making strict HBV monitoring mandatory after stopping treatment. Furthermore, the 100 mg dose used for HBV is completely inadequate for treating HIV, meaning mandatory HIV testing is required before initiation to avoid creating subtherapeutic HIV resistance. The medication also carries warnings for dangerous conditions such as lactic acidosis, pancreatitis, and severe hepatomegaly with steatosis. Finally, there is a distinct warning regarding the high likelihood of developing HBV resistance when using this medication as a long-term monotherapy.

Avoid concurrent use with HIV products containing higher-dose lamivudine (Epivir 300 mg, Combivir, Trizivir, Epzicom) — combining would result in excessive lamivudine exposure.
