Delstrigo (DOR/3TC/TDF) is a once-daily, single-tablet, three-drug HIV-1 regimen combining the NNRTI doravirine with the NRTI backbone of lamivudine and tenofovir disoproxil fumarate (TDF).
How Delstrigo Works:
HIV-1 replication requires reverse transcriptase to convert viral RNA into DNA. Delstrigo attacks reverse transcriptase from two complementary directions.
Doravirine is an NNRTI — it binds reverse transcriptase at a non-active (allosteric) site, distorting the enzyme. Lamivudine and tenofovir are NRTIs — they get incorporated into the growing viral DNA chain and terminate elongation. Together, this three-drug combination achieves effective viral suppression. Delstrigo uses tenofovir disoproxil fumarate (TDF) — older than TAF but with similar antiviral activity (with slightly higher kidney and bone exposure).
Form and use:
Delstrigo is taken as one tablet by mouth once daily, with or without food. It is a complete regimen — no other antiretrovirals are needed. Unlike rilpivirine-based regimens, Delstrigo does not require a meal and does not interact with acid-reducing medications.
Generic availability:
There is no generic Delstrigo in the U.S. as of 2026. Doravirine is also available alone (Pifeltro) for combining with different NRTIs. Other complete single-tablet regimens include Biktarvy (BIC/FTC/TAF), Genvoya (EVG/c/FTC/TAF), Triumeq (DTG/ABC/3TC), Dovato (DTG/3TC, two-drug), and Symtuza (DRV/c/FTC/TAF). HIV guidelines generally prefer integrase-inhibitor-based regimens for treatment-naïve patients.
Warnings:
Delstrigo carries a BOXED WARNING for severe acute HBV exacerbation if discontinued in patients with HBV co-infection — HBV testing is required before starting. Other warnings include lactic acidosis/severe hepatomegaly with steatosis, new-onset or worsening renal impairment (TDF is more nephrotoxic than TAF), decreased bone mineral density (TDF), hepatotoxicity, depression/suicidality (rare), and immune reconstitution syndrome. Significant drug interactions: rifampin and other strong CYP3A inducers substantially reduce doravirine levels; anticonvulsants and St. John’s wort also reduce doravirine.

TDF has been widely used for many years and is effective, though it has slightly higher rates of kidney and bone effects than the newer TAF formulation. The decision between TDF and TAF often comes down to baseline kidney function, age, and bone health considerations.
Yes. Rifampin substantially reduces doravirine levels (avoid). Rifabutin requires the doravirine dose to be increased to twice daily. Anticonvulsants (carbamazepine, phenytoin, phenobarbital) and St. John’s wort reduce doravirine. Unlike rilpivirine-based regimens, Delstrigo does NOT interact significantly with PPIs, H2 blockers, or antacids — a practical advantage.
Delstrigo’s lamivudine and tenofovir both have activity against HBV. Tell your prescriber if you have HBV co-infection — you should not stop Delstrigo without HBV monitoring, as severe HBV flares can occur. HBV testing is required before starting.
Yes. Medicare Part D beneficiaries can typically qualify, especially if you face specialty-tier copays. State ADAP programs may also coordinate with Medicare for HIV-treatment patients.
