Understanding Cuprimine:

Cuprimine (penicillamine) is a copper chelator used to treat Wilson’s disease — a rare autosomal recessive disorder of copper metabolism causing copper accumulation in liver, brain, and other organs. Untreated Wilson’s is fatal; lifelong chelation therapy prevents complications.

How Cuprimine Works:

Wilson’s disease results from ATP7B mutations impairing biliary copper excretion. Copper accumulates causing hepatic, neurologic, psychiatric, ophthalmologic (Kayser-Fleischer rings), and other complications. Lifelong copper-reducing therapy is essential.

Penicillamine forms soluble complexes with copper that are excreted in urine. Reduces tissue copper burden over months. Initial therapy uses penicillamine or trientine to remove copper; maintenance can use zinc to block intestinal copper absorption.

Form and use:

Cuprimine taken by mouth on empty stomach (1 hour before or 2 hours after meals) and at least 1 hour from other medications. Pyridoxine (vitamin B6) 25 mg/day supplement recommended.

Generic availability:

Generic penicillamine widely available.
