Multiple myeloma cells express GPRC5D on their surface far more than healthy cells do. Talquetamab bridges those cells directly to T-cells, forcing an immune attack that doesn’t depend on the exhausted, heavily pretreated immune system finding the cancer on its own. It’s approved for patients who’ve been through at least four prior lines of therapy — the population with the fewest options — and targets a different antigen than BCMA-directed therapies, so it can work after they fail.
Form and Use:
Given as a subcutaneous injection by healthcare providers — never self-administered — starting with step-up doses that acclimate the immune system, each followed by a recommended 48-hour hospitalization to monitor for cytokine release syndrome. Maintenance is weekly or every two weeks, at a REMS-certified site. The financial architecture follows the clinical one: drug cost, administration fees, and step-up hospital stays are three different bills with three different assistance answers — which is why the coordination matters as much as any single program.
Biosimilar Availability:
None exists or is on any near-term horizon — talquetamab was approved in 2023 and holds long exclusivity as a first-in-class biologic.
