How It Works:
Myelofibrosis is a chronic myeloproliferative neoplasm driven by JAK2 V617F (~60% of patients), CALR (~25%), or MPL (~10%) mutations — all leading to constitutive JAK/STAT signaling. Manifestations include splenomegaly, constitutional symptoms (fatigue, night sweats, weight loss, fevers), progressive bone marrow fibrosis, cytopenias (especially anemia and thrombocytopenia), and risk of leukemic transformation. Fedratinib selectively inhibits JAK2 and FLT3 — blocks constitutive JAK/STAT signaling driving myeloproliferation. Reduces splenomegaly, improves constitutional symptoms, and provides clinical benefit in JAK inhibitor-naive patients (JAKARTA) and after ruxolitinib failure/intolerance (JAKARTA-2). Does not modify the underlying clonal disease or prevent disease progression — allogeneic SCT remains the only potentially curative option.
Form and Use:
Inrebic 100 mg capsules. Recommended dose 400 mg (4 capsules) once daily. Take with or without food (high-fat meal may reduce nausea). Continue until disease progression or unacceptable toxicity. Mandatory baseline thiamine (vitamin B1) level — supplement if deficient before starting, due to boxed warning for encephalopathy including Wernicke’s encephalopathy. Monitor thiamine periodically and supplement. Periodic CBC, comprehensive metabolic panel including amylase/lipase, liver enzymes. Dose modifications for cytopenias, hepatic, pancreatic, and other toxicities.

Warnings:
Boxed warning: serious and fatal encephalopathy, including Wernicke’s encephalopathy — caused by thiamine deficiency exacerbated by Inrebic; mandatory baseline thiamine assessment and supplementation if deficient before starting; ongoing monitoring and supplementation; immediate discontinuation if encephalopathy suspected (mental status changes, ataxia, ocular abnormalities). Other: anemia (very common, dose-limiting — may require transfusions), thrombocytopenia and bleeding (monitor; hold for severe; concurrent anticoagulant/antiplatelet caution), gastrointestinal toxicity (diarrhea, nausea, vomiting, constipation — manage proactively), hepatic toxicity (monitor LFTs; hold for severe), amylase/lipase elevation and pancreatitis (monitor), increased infection risk (PML/JC virus reactivation reported — rare), embryo-fetal toxicity. Drug interactions: strong dual CYP3A4 inhibitors (e.g., ketoconazole) increase fedratinib — reduce dose; strong CYP3A4 inducers (rifampin, phenytoin, carbamazepine, St. John’s wort) reduce efficacy — avoid.
