How It Works:
COPD involves both bronchoconstriction and airway inflammation. Standard therapy uses inhaled bronchodilators (SABAs, LABAs, LAMAs) and inhaled corticosteroids. The oral PDE4 inhibitor (roflumilast) provides anti-inflammatory benefit for severe COPD with chronic bronchitis. Novel therapy targets remained an unmet need. Ensifentrine is a dual PDE3 and PDE4 inhibitor. PDE3 inhibition causes airway smooth muscle relaxation (bronchodilation). PDE4 inhibition reduces inflammatory mediators. Combined mechanisms provide bronchodilation and anti-inflammatory effects in one nebulized therapy — an add-on to standard inhaler-based COPD therapy that established improved lung function and reduced exacerbations vs placebo in phase 3 trials.
Form and Use:
Ohtuvayre nebulized inhalation suspension. Twice daily via jet or vibrating mesh nebulizer. Individual dose ampules. Add-on to existing COPD inhaler therapy.
Generic Availability:
No generic Ohtuvayre. Other nebulized COPD therapies: albuterol (generic short-acting rescue), levalbuterol (Xopenex generic), formoterol (Perforomist generic — long-acting), Yupelri (revefenacin nebulized LAMA — brand).
