It was specifically designed as a once-daily nebulized LAMA — addressing the need for once-daily nebulized therapy in patients who cannot use handheld LAMA inhalers.
How Yupelri Works:
COPD bronchoconstriction occurs when acetylcholine binds to muscarinic M3 receptors on airway smooth muscle, causing contraction and narrowing. Revefenacin is a long-acting muscarinic antagonist (LAMA) that blocks these M3 receptors to prevent narrowing and provide 24-hour bronchodilation. Delivered once daily via a nebulizer, it offers an advantage over handheld inhalers for patients with poor inhaler technique, low inspiratory flow, cognitive impairment, or severe arthritis. However, the trade-off is the extra time and equipment burden required for nebulizer treatments.
Form and use:
Yupelri is administered once daily as a single 175 mcg/3 mL unit-dose vial. The medication must be inhaled using a standard jet nebulizer, with each treatment session lasting approximately 8 minutes. For consistency, patients should administer the dose at the same time every single day. Additionally, Yupelri should never be mixed with other nebulizer medications within the same nebulizer cup.
Generic availability:
As of 2026, there is no generic version of Yupelri available in the United States. Other LAMA options for COPD include umeclidinium, aclidinium, glycopyrrolate (which offers another nebulized option), and tiotropium, which has a generic version available for the HandiHaler. For patients who possess the ability to properly use handheld devices, generic Spiriva HandiHaler or alternative LAMA inhalers are typically more cost-effective than Yupelri.
Warnings:
Yupelri carries specific warnings for paradoxical bronchospasm, hypersensitivity reactions, and the worsening of acute angle-closure glaucoma. Due to the risk of urinary retention, it should be used with caution in patients with BPH or narrow-angle glaucoma, and avoided by those with severe sensitivities to similar drugs. As a LAMA, this medication is not intended for asthma treatment or for the relief of acute bronchospasm. Because revefenacin has limited systemic absorption, its drug interactions remain minimal.

Don’t mix Yupelri and albuterol in the same nebulizer cup unless your prescriber confirms compatibility — run them sequentially.
Many COPD patients benefit from combined LABA + LAMA therapy (Anoro Ellipta, Stiolto Respimat, Bevespi Aerosphere). If you have access to use these inhalers well, combination LABA/LAMA may be more convenient (single inhaler) and similarly effective. Yupelri is a single-class agent (LAMA only); some patients also need a LABA. Discuss the optimal regimen with your pulmonologist.
Tell your pulmonologist. LAMAs can worsen narrow-angle glaucoma and urinary retention from benign prostatic hyperplasia (BPH). Mild BPH usually does not preclude LAMA use, but discuss before starting. Tell your prescriber about new eye pain (could indicate acute angle-closure glaucoma) or urinary difficulty.
