Zepatier is direct-acting antiviral (DAA) for chronic HCV genotypes 1 and 4. Highly effective — cure rates 95%+ in most patients. Era of HCV DAAs (since 2014) has revolutionized HCV treatment — most patients now achievable cure with 8–12 week oral regimens.
How Zepatier Works:
HCV replication requires viral proteins NS5A (replication complex assembly) and NS3/4A (protease — viral polyprotein processing). Direct-acting antivirals target these proteins.
Elbasvir is NS5A inhibitor. Grazoprevir is NS3/4A protease inhibitor. Combination provides robust antiviral effect — cure rates 95%+. Resistance-associated substitutions (RAS) testing recommended for genotype 1a; presence of NS5A RAS may require ribavirin addition or alternative regimen.
Form and use:
Zepatier 1 tablet once daily for 12 weeks (some patients 16 weeks with ribavirin).
Generic availability:
No generic Zepatier. Other HCV DAAs with authorized generics: Epclusa (sofosbuvir/velpatasvir authorized generic — pangenotypic 12-week), Harvoni (ledipasvir/sofosbuvir authorized generic — genotype 1). Mavyret (glecaprevir/pibrentasvir — pangenotypic 8-week) brand. Choice based on genotype, treatment history, cirrhosis status, drug interactions.
Warnings:
Hepatitis B reactivation (screen for HBV before HCV treatment; if HBsAg+ or HBcAb+, monitor or co-treat HBV), ALT elevations (especially in cirrhotic patients with NS5A polymorphisms), drug interactions (CYP3A — many; OATP transporter), contraindicated in moderate/severe hepatic impairment.
